  7. revisions left by other means will be incorporated into the

  10. When leaving information, be sure to COMPLETELY fill out the
  11. questionnaire even if you are only changing certain
  12. information. This allows the editor to verify current
  13. information. NOTE: Please include the (OLD) PHONE NUMBER
